JAMA:美国真实世界的急性缺血性卒中血管内治疗大数据来了

随机对照试验显示对于大血管闭塞性急性缺血性卒中(acute ischemic stroke,AIS)采取血管内再灌注治疗优于内科治疗。几项研究提示获益具有时间依赖性,越早治疗获益越大。关于从发病到治疗的时间与预后之间的关系,现有数据受到了精确度和代表性的限制。随机对照试验汇总分析的样本量也不大,5项试验中仅纳入了536例血管内再灌注治疗的患者,因此随机对照试验的结果不一定能够直接推广到常规临床实践


随机对照试验显示对于大血管闭塞性急性缺血性卒中(acute ischemic stroke,AIS)采取血管内再灌注治疗优于内科治疗。几项研究提示获益具有时间依赖性,越早治疗获益越大。关于从发病到治疗的时间与预后之间的关系,现有数据受到了精确度和代表性的限制。随机对照试验汇总分析的样本量也不大,5项试验中仅纳入了536例血管内再灌注治疗的患者,因此随机对照试验的结果不一定能够直接推广到常规临床实践中。观察性研究也受到效能的限制,无法精确反映时间效应。

2019年7月来自美国的Reza Jahan等利用美国国家跟着卒中威廉亚洲博彩公司 走(GWTG-Stroke)的数据,探讨了血管内-再灌注治疗的时间与预后之间的关系。

共6756例患者,平均年龄69.5岁,51.2%为女性,治疗前平均 NIHSS 评分为17(IQR 12-22)分。发作到穿刺平均时间为230min(IQR 170-305),进门到动脉穿刺时间为87min(IQR 62-116),mTICI 2b-3占85.9%。

sICH 占 6.7%,在院死亡/出院到临终关怀医院(hospice discharge)占19.6%。出院时,不卧床自理占36.9%,mRS 0-2占23%。

在从发作到穿刺的校正分析中,30min 到270min 比271min 到480min 的时间-预后相关性更加陡峭非线性;在30min 到270min 时间窗内,以15min 为增量从发作到穿刺的时间越快,出院时达到独立步行的可能性越大,在院死亡率/出院到临终关怀医院的比例越低,sICH 的几率越小。进门到动脉穿刺的时间也与预后改善有着类似的关系,包括在30min 到120min 的时间窗,出院回家的可能性更高,在于死亡率/出院到临终关怀医院的的比例更低(按照15min 的增量计算)。

最终作者认为对于大血管闭塞性急性缺血性卒中患者,在常规实践中血管内再灌注治疗的时间越短,功能预后越好。
