肺部炎症常见的“七大”CT表现

小斑片状或斑片状的融合影可见各种原因的肺炎。直径为1.0～2.5cm．边缘模糊。

​1、小结节影

 

常见于腺病毒，也可见于细菌，真菌感染。病灶一般在5～6mm，边缘较模糊，以中下肺野为多也可有与小叶或小叶融合病灶并存。常为多发（图1）。

2、小斑片状或斑片状的融合影

可见各种原因的肺炎。直径为1.0～2.5cm．边缘模糊。斑片影并可见发生融合，一般表现为：下比上多，内比外多，后比前多（图2）。

3、肺段及大叶阴影

多见于细菌感染，也可见于腺病毒和卡氏囊虫感染。以肺段或叶为界的大片实变，体积不缩小，内有支气管充气征，边缘常较淡，模糊(图3)。

4、条索状及网状阴影

此征常见于放射性肺炎，慢性肺炎，间质性肺炎和肺炎的吸收期。病变为增殖为主，也可合并有实质性肺泡炎。表现为斑片状不规则的条索状混合影。边缘可清楚，也可模糊（图4，图5）。

5、球形阴影

见于金黄色葡萄球菌肺炎，真菌性肺炎，炎性假瘤。金黄色葡萄球菌肺炎，多发或单发的球形影，多为1—3cm边缘较清晰，密度较均匀，增强时为周围强化，但易变，且常合并空洞：真菌性肺炎是脓肿与肉芽肿，变化慢；炎性假瘤，大小不一，边缘可光整，毛刺，增强可均匀或混合强化（图6，图7）

6、空洞
常见于化脓性肺炎或真菌性肺炎。见环形的透亮区，边缘可清晰，可模糊。壁可厚可薄。常伴有斑片状模糊影或小结节影（图8）。

7、胸膜病变

合并积液，后期胸膜局部增厚。

支气管起源病变，诊断是？

患者，男性，74岁，体检发现左肺阴影1周

咳、咳、咳个不停，还在当支气管炎来治？

最近接诊了多例咳嗽迁延不愈的患者，他们曾到过多个门诊反复就医，都被诊断为感冒、支气管炎，但在反复使用抗菌药物和各种止咳药物治疗后，咳嗽依然没有好转。

探案：反复咯血的始作俑者是曲霉吗？

2016年2月底无明显诱因下咯血，持续数天，每日咯血量约100-200ml，无咳嗽咳痰、胸闷气促、发热盗汗、乏力纳差等症状。2016-03-01当地医院胸部CT示：左下肺支气管闭塞，右下肺磨玻璃小结节，右肺中叶小结节。CRP 9.9mg/L，ESR 45mm/H，痰找抗酸杆菌*2次：阴性，T-SPOT阴性（具体不详）。

NEJM：支气管黑色素瘤-病例报道

体格检查发现左肺明显的吸气和呼气喘鸣。胸部计算机断层扫描显示息肉样病变，并且阻塞左主干支气管。硬性支气管镜检查发现支气管中移动性色素沉着肿块。用电烙套管和氩等离子体凝固清除肿块。肿块的病理检查证实了转移性黑素瘤的诊断，伴有BRAF突变。
