CMI:注射卡介苗疫苗可以减少新冠肺炎疫情期间医护人员的缺勤率

卡介苗(BCG)广泛用于预防结核病,也对其他传染病提供非特异性效应。

背景:新冠肺炎大流行是由严重急性呼吸综合征冠状病毒-2(SARS-CoV-2)引起的,是护理新冠肺炎患者的医护人员发病的一个重要原因。此外,新冠肺炎患者的负担给世界各地的医疗保健系统能力带来了巨大的压力。在疫情初期,当针对SARS-CoV2的有效疫苗的有效性和及时性尚未确定时,防止通过其他手段防止HCW旷工是保障患者护理的当务之急。建议将非特异性疫苗作为降低新冠肺炎发病率和严重程度的一种手段。

卡介苗(BCG)广泛用于预防结核病,也对其他传染病提供非特异性效应。这些非特异性效应导致对异种病原体的天然免疫反应更快、更有效,这一过程被称为“训练免疫”。体外和体内都观察到对不同的病毒病原体,如呼吸道合胞病毒、流感病毒和减毒黄热病疫苗病毒的有益效果。

目的:新冠肺炎疫情增加医护人员(HCW)缺勤率。卡介苗(BCG)疫苗可以通过增强训练免疫来提供针对呼吸道感染的非特异性保护。williamhill asia 调查了新冠肺炎疫情期间卡介苗接种对HCW旷工的影响。

方法:在9家荷兰医院中接触过新冠肺炎患者的hcw以1:1的比例随机接种卡介苗或安慰剂,并使用手机应用程序随访一年。主要终点是自我报告的任何原因的计划外缺勤天数。次要终点包括记录的新冠肺炎、急性呼吸道症状或发热。这是一项由研究者资助的研究,在ClinicalTrials.gov注册(NCT03987919)。

结果:2020年3月/4月,有1511名医务工作者入选。随访持续时间的中位数是357人天(IQR,351至361)。在卡介苗组的2.8%的计划工作日和安慰剂组的2.7%的计划工作日中观察到任何原因的计划外缺勤(调整后的相对风险为0.94,95%可信区间(CI)为0.78至1.15)。卡介苗组和安慰剂组记录的新冠肺炎累积发生率分别为14.2%和15.2%(校正风险比(aHR)为0.94,95%可信区间为0.72-1.24)。分别有490名(66.2%)和443名(60.2%)受试者首次出现自述急性呼吸道症状或发热(aHR: 1.13,95% CI 0.99至1.28)。报告了31起严重不良事件(13起发生在接种卡介苗后,18起发生在安慰剂后),无一例被认为与研究药物有关。

结论:在新冠肺炎疫情期间,暴露于新冠肺炎患者的HCW接种卡介苗后没有减少无计划旷工。
