Neurology:多发性硬化患者皮层萎缩会加重认知功能下降

本研究为了验证多发性硬化病程中加速认知功能损害的病理过程,利用纵向MRI进行研究,其对复发缓解型MS(RRMS)和进展性MS(PMS)的认知下降具有诊断意义。

方法

本研究纳入了230例MS患者和59例健康志愿者进行评价,其中共有22例RRMS患者转化为PMS患者。利用纵向MRI数据计算没见皮层和深部灰质萎缩率和病灶体积变化,并在相同时间利用认知量表评价认知功能下降情况,评价两者的相关性。

结果

在PMS和RRMS患者中深部灰质萎缩率并无差异(-0.82%/year vs -0.71%/year, p = 0.11),而PMS皮层萎缩要快与RRMS患者(-0.87%/year vs -0.48%/year, p < 0.01)。同样,PMS患者较RRMS患者认知功能下降更快(p < 0.01)。每年认知功能下降与稳定性RRMS患者的病灶体积增加(r = -0.17, p = 0.03)、转化的RRMS患者的每年深部灰质体积萎缩率(r = 0.50, p = 0.02)以及PMS每年皮层萎缩(r = 0.35, p = 0.01)具有相关性。

结论

本研究结果表明在MS进程中皮质萎缩和认知下降是共存的。认知下降的在稳定性RRMS患者中体现在局部病灶的病理改变,在转化中RRMS患者中体现为深部灰质萎缩,而在PMS患者中体现在皮层萎缩加速。
