研究招募了30名T1D≥3年的参与者。首先,参与者佩戴盲目的CGM装置进行7天的自由生活数据采集。其次,3小时混食试验评估刺激C肽和胰岛素。峰值C-肽用于将参与者分配到检测不到(Cpepund <3 pmol/L)、低(Cpeplow 3-200 pmol/L)或高(Cpephigh >200 pmol/L)C-肽组。最后,参与者在60%VO2peak下完成45分钟的倾斜跑步机行走,然后再进行48小时CGM采集。
结果,在自由生活观察周,各组CGM参数具有可比性。在运动后的12 h和24 h,与Cpeplow(12 h,43.6 ± 26.1%,P = 0.027;24 h,52.3 ± 25.0%,P = 0.067)或Cpepund(12 h,40.6 ± 17.0%,P = 0.010;24 h,51.3 ± 22.3%,P = 0.041)相比,Cpephigh组在葡萄糖3.9-10 mmol/L的时间明显增多(12 h,73.5 ± 27.6%;24 h,76. 3 ± 19.2%)。与Cpepund和Cpeplow相比,Cpephigh组在高血糖中花费的时间(12 h和24 h血糖>10和>13.9 mmol/L,P <0.05)和血糖变异性(12 h和24 h SD,P <0.01)显著降低。CGM结果从运动前到运动后24 h的变化不同:Cpepund和Cpeplow经历了恶化(血糖3.9-10 mmol/L:分别为-9.1%和-16.2%),Cpephigh经历了改善(+12.1%)(P = 0.017)。
综上所述,该研究结果表明,残余β细胞功能可能部分解释了T1D患者运动急性降糖益处的个体间差异。量化C肽可以帮助为运动患者提供个性化和有针对性的支持。
