Heart:女性补钙要抓紧,骨质疏松或将影响心血管健康!

骨质疏松大家都非常熟悉,它如今已经被世卫组织列为“第二大危害人类健康的疾病”,仅次于心血管疾病,随着年龄的增长,人们患病风险也会不断增加。

近日,来自韩国首尔大学的研究团队在《Heart》杂志发表了一篇题为 Prognostic value of lower bone mineral density in predicting adverse cardiovascular disease in Asian women 的文章,通过分析超1.2万亚洲女性的数据,揭示了骨密度低或者骨质疏松与心血管疾病的潜在关联。

在此项研究之前,曾有研究发现因心血管疾病而死亡的女性(21%)群体概率明显高于男性(15%),同时,传统意义上包括年龄、BMI、高血压、二型糖尿病等临床风险因素在影响男性与女性心血管疾病时存在区别,更适用于男性。因此,为了更好地预测女性心血管疾病(ASCVD)的发作,此次研究将骨密度(bone mineral density,BMD)与骨质疏松加入到临床风险因素当中,以求探索低骨密度是否与女性心血管疾病存在强关联。

研究人员首先浏览了2005年到2014年之间首尔大学盆唐医院50-80岁女性的双能X线吸收测定(dual-energy X-ray absorptiometry,DXA)记录,在排除了做DXA扫描前已经发生心血管或其他重大疾病的案例后,最终筛选出了12681名女性进行了分析调查。

这些参与者平均年龄为63岁,在长达9年的中位随访期内,468名(约4%)女性遭遇了ASCVD,237名(2%)女性因ASCVD而死亡。

研究人员使用比例风险回归模型来评估ASCVD发作的风险比率,他们根据传统风险因素以及BMD与骨质疏松等因素进行了变量分组,并将这些变量融入模型中,从而分析调整后的模型是否能够帮助人们更好地预测ASCVD的发作风险比率。数据分析显示,当综合考虑BMD与骨质疏松变量以及传统因素变量后,参与者显示出更高的ASCVD发作风险。

风险因素加入BMD与骨质疏松症后的ASCVD发作风险比率

为了验证BMD与骨质疏松症的加入是否能够帮助人们更好地预测ASCVD的发作,研究人员建立了单一年龄模型(Model1)、传统风险因素模型(Model 2)、BMD模型(Model 3)以及骨质疏松症模型(Model 4)四种预测模型,并采取逐渐叠加变量的形式进行两两比较,以此来判断哪一款模型在预测ASCVD时最优。

所得数据显示,Model 2 要比 Model 1的预测效果更好,Model 3 在预测时要比 Model 2 更有效,同样,Model 4 的预测效果也比Model 2 的更好。

在进一步试验中,研究人员将Model 4 中的骨质疏松症明确为髋关节骨质疏松,并将其与ASCVD 10年风险数值预测模型进行比较,结果显示,Model 4 更胜一筹,展现了更优的预测能力。这说明,当BMD或者骨质疏松症加入至风险因素进行预测时,确实会比传统的预测模型得到更好的预测效果。

首尔大学的这一研究成果证实了女性低骨密度与患心血管疾病存在密切关系,但是需要注意的时,两者究竟存在怎样的因果关系还有待进一步的研究证明。不过,通过这项研究,或许会为关爱与治疗女性心血管疾病方面打开新的突破口。
