Eur Heart J:注意啦!亚洲沙尘事件可能诱发急性心梗

起源于中国和东亚沙漠中矿物土壤的风沙沙尘被称为亚洲沙尘(即“黄沙”)。虽然亚洲沙尘一直被认为是一种自然现象,但由于流行病学研究表明其与疾病和不良健康具有潜在相关性,近来它已成为一种严重的卫生保健问题。2017年发表在《Eur Heart J》的一项由日本科学家进行的研究,考察了亚洲沙尘暴露是否诱发急性心肌梗死(AMI)。
目的:阐明亚洲沙尘是否与AMI发病率相关,并弄清楚对亚洲沙尘高敏感的患者是否进展为AMI。
方法和结果:日本熊本县21家有能力开展冠脉介入的机构被纳入本研究。数据为在熊本县气象台检测亚洲沙尘的地面观测数据。2010年4月1日~2015年3月31日间收集的数据被分析,3713名连续的AMI患者被纳入。采用时间分层病例交叉设计考察亚洲沙尘暴露和AMI之间的相关性。AMI发病前1日的亚洲沙尘事件发生率与AMI发病率相关[比值比(OR),1.46;95%置信区间(CI),1.09~1.95],尤其非ST段抬高心肌梗死更为明显(OR 2.03;95%CI,1.30~3.15)。在年龄≥75岁、男性、高血压、糖尿病、不吸烟和慢性肾病(CKD)患者中,AMI与亚洲沙尘显着相关。但是,亚洲沙尘事件对CKD患者的AMI发病具有较大的作用(P<0.01)。一个考虑许多AMI风险因素的评分系统被开发。亚洲沙尘事件的发生率与风险评分为5~6的患者AMI发病率明显相关(OR 2.45;95% CI:1.14~5.27)。
结论:亚洲沙尘事件可能导致AMI,并且对CKD患者的AMI发病具有较大的影响。
