这项队列研究纳入了从2009年1月到2014年12月期间在全国性的电子健康记录库确定患者,包括多个供应商网络和电子医疗记录系统。来自于结构化数据领域和自由文本临床记录的低血糖指标被归类为严重或不严重两类。协变量调整后的泊松回归分析被用来评估低血糖的频率(严重程度)或体重改变大小与急性心肌梗死(AMI)、慢性充血性心力衰竭(CHF)和卒中发病率之间的相关性。
在符合入选标准的143635例患者中,共观察到5669例AMI患者,14109例充血性心力衰竭患者和7017例卒中患者。其中AMI、CHF和卒中的总发病率分别为1.53、4.26和1.92人每100人每年。总体低血糖与每个CVD结局之间呈正相关性,在严重低血糖患者中可见到更强的相关性,在非严重低血糖患者中可见到衰减的或无相关性。体重变化与心血管疾病风险呈U形相关性,相比于体重平稳,体重增加和体重减轻均与风险增加相关。
