Cell Res :突破!上海科技大学张辉团队发现心内膜中Kdr的过度表达可改善心肌梗死后的心脏功能

血管内皮生长因子(VEGF)及其受体(VEGFRs)已显示在血管生成中起重要作用。因此,该研究探索了VEGFR2(也称为KDR)在不同发育阶段在心内膜中的表达。该研究发现在胚胎(E)第13.5天,E16.5和出生后第(P)1天的心内膜中可以检测到KDR,并且心内膜KDR的表达从E13.5逐渐降低到P1。但是,在P7或成年期的心内膜中未检测到明显的KDR表达。考虑到成人心内膜中KDR表达的降低,并考虑到成人而不是胎儿心内膜丧失了新生血管形成能力,研究人员推测成人心内膜中的KDR再次引入可能会重新激活其血管生成的发育程序并促进MI后的新生血管形成。

因此,为了在成年心内膜中特异性表达KDR并同时追踪心内膜细胞,该研究通过插入CAG启动子驱动的表达盒产生了诱导型小鼠H11-loxp-stop-loxp-Kdr-2A-mCherry(H11-lsl-Kdr-mCherry)将loxp-stop-loxp-Kdr-2A-mCherry插入位于Eif4enif1和Drg1基因之间的基因间区域内的Hipp11(H11)基因座中。首先,将H11-lsl-Kdr-mCherry小鼠与特异心内膜Cre驱动的Npr3-CreER mice杂交,在出生后8周(P8W)用他莫昔芬处理了Npr3-CreER; H11-lsl-Kdr-mCherry小鼠,并在48 h后收集心脏进行分析。对KDR,mCherry和CDH5进行的免疫染色显示,在心内膜中可以检测到KDR和mCherry,这表明在成年 Npr3-CreER;H11-lsl-Kdr-mCherry的小鼠心内膜中成功表达了KDR。

接下来,该研究检查了心脏动态平衡期间表达Kdr的心内膜的细胞命运。他莫昔芬在P8W给药,并在1、10或26周后收获心脏进行分析。在P9W Npr3-CreER; H11-lsl-Kdr-mCherry小鼠中未鉴定出mCherry标记的冠状VEC;他莫昔芬治疗后10周,仅在心肌中检测到稀疏的mCherry +冠状动脉VEC。尽管如此,他莫昔芬治疗后半年,仍然跟踪过Kdr过表达的心内膜的命运,并有趣地在心肌自由壁的内层中发现了一个标有mCherry的巨大冠状VEC。总的来说,具有长时间心内膜Kdr过表达在心脏动态平衡期间有助于冠状VEC。

为了解决在心肌梗死期间过表达Kdr的心内膜是否有助于新的冠状动脉VEC,研究发现MI诱导的VEGFa可能触发Kdr过表达的心内膜转化为受损心脏中的冠状VEC。接下来,该研究进行了天狼星红染色以检查MI后的心脏纤维化,发现与对照组相比,心内膜过表达Kdr明显减少了纤维化区域。同时,在过表达Kdr的心脏的梗塞区域中内皮密度增加,心肌细胞凋亡减少。因此,成年心内膜中Kdr的过表达减少了心肌纤维化,增加了血管密度,减少了心肌细胞凋亡,并改善了MI后的心脏功能。

在临床前研究中,已报道给予重组VEGFa蛋白或病毒介导的VEGFa过表达可促进心血管形成或改善心脏功能。然而,迄今为止,VEGFa的临床试验尚未显示出任何临床益处,这可能是由于VEGFa治疗的副作用,VEGFa蛋白的寿命短,转染不足或其他原因所致。在该研究中,发现MI后成年小鼠心内膜KDR的过度表达促进了心内膜向VEC的转化,这表明梗死区域内源性KDR配体足以诱导心内膜新生血管,而无需额外供应VEGF蛋白。

总之,该研究工作表明,心内膜过表达Kdr在心脏稳态和MI后将成人内膜转化为冠状血管,从而改善缺血后的心脏功能。临床相关的可应用策略以促进心内膜新生血管的发展值得进一步研究。
