PNAS:雷公藤变成为靶向药物?有望用于治疗IDH1突变的癌症

这篇论文于近期发表在《美国科学院院刊》上,论文的通讯作者为中科院大连化学物理研究所的许国旺研究员和美国国家癌症研究所的杨春章研究员。研究者们发现, 雷公藤( Tripterygium wilfordii )的主要活性成分,雷公藤甲素,能够通过抑制Nrf2相关的谷胱甘肽合成通路,让 IDH1 突变的癌细胞死于氧化应激 [1]。

雷公藤首次被详细记载时,就以毒药的形式出现。清代的《本草纲目拾遗》记载道:“采之毒鱼,凡蚌螺亦死,其性最烈,以其草烟獯蚕子则不生。”

这对于IDH1抑制剂久久不能拿下的神经胶质瘤等癌种来说,无疑是个大大的新希望

异柠檬酸脱氢酶( IDH1/2 )突变 是非常常见的癌症基因突变,在神经胶质瘤、急性髓系白血病、胆管癌、软骨肉瘤、胰腺癌中都有发生。 IDH1 的突变会使得酶活性发生变化,不巧的是IDH还是代谢中非常关键的酶,这就使得 IDH1 突变会直接影响到三羧酸循环、氧化平衡和能量代谢。临床中,约有6%~10%的AML患者存在异柠檬酸脱氢酶(IDH)1突变。

事实上,williamhill asia 医学此前也有报道,见:STM:中药成分雷公藤甲素可抗胰腺癌AJP:雷公藤提取物通过激活内质网应激杀死胰腺癌细胞,但是,这篇文章直接将雷公藤甲素引向“靶向药物”的可能。

另外,靶向IDH1的抑制剂也早有开发,在血液肿瘤中表现出了不错的战绩,2018年Agios公司研发的靶向新药Tibsovo(ivosidenib)已获FDA批准,用于易感异柠檬酸脱氢酶-1(IDH1)突变的复发性或难治性急性髓系白血病(R/R AML)成人患者。另外,Tibsovo的兄弟药物Idhifa(Enasidenib)--首款IDH2抑制剂,已于2017年8月获FDA批准用于治疗IDH2变异的复发性/难治性急性髓系白血病。另外,ivosidenib,在胆管癌lll期临床试验ClarlDHy中显示疗效显著,ivosidenib可以显著提高胆管癌患者的无进展生存期PFS)2.7个月,总生存期延长至10.8个月,死亡风险降低63%。不过,这两种药物在神经胶质瘤和软骨肉瘤等实体瘤中却响应不佳

有什么办法攻克这个难关呢?这就要看看IDH1突变会给细胞带来什么变化了。

研究者们首先用强力霉素(Dox)诱导出了细胞的 IDH1 突变,结果发现 在突变发生24小时后,细胞中的活性氧(ROS)水平大大增加了,这使得脂质等大分子的氧化水平也增加了。在来自患者的脑肿瘤细胞中,研究者们也发现了类似的氧化应激现象。

氧化应激这事儿有利有弊,它能协助杀死侵入细胞的病原体,但氧化水平过高,也会反过来让细胞跟着一起“殉情”。williamhill asia 有没有办法利用一下 IDH1 突变癌细胞的这个特征,让它自作孽不可活呢?

当然细胞肯定也不傻,它们自有一套抗氧化的手段,其中就包括由 核因子相关因子2(Nrf2)调控的著名抗氧化物,谷胱甘肽

果然,研究者发现 携带 IDH1 突变的细胞中, Nrf2 转录活性提高了2-3倍 ,其下游与抗氧化有关的基因,例如 GCLC、GCLM、SLC7A11、NQO1 等都非常活跃,说明 Nrf2 调控的抗氧化通路明显被激活了 

值得注意的是, 其中 SLC7A11 上调得最明显,与 IDH1 正常的细胞相比,表达水平高了7倍!

这个 SLC7A11 ,不出所料,就是细胞从头合成谷胱甘肽的关键!正是因为 SLC7A11 的存在, IDH1 突变细胞才免于凋亡。

现在轮到雷公藤甲素登场了。 雷公藤甲素(Triptolide, TPL)是一种环氧二萜内酯化合物,能在纳摩尔水平上表现出强大的转录抑制活性。比较令人惊喜的是,研究者们发现 受雷公藤甲素抑制的基因们,竟然大量与 Nrf2 的下游基因重合。没错,包括 SLC7A11 。

接下来就非常顺理成章啦。 通过雷公藤甲素,Nrf2介导的谷胱甘肽合成被抑制,IDH1突变细胞内升高的ROS无人应对,逐渐导致了DNA和脂质等大分子的氧化损伤,并最终带来细胞凋亡。

雷公藤甲素又称雷公藤内酯、雷公藤内酯醇,是从卫矛科植物雷公藤的根、叶、花及果实中提取的一种环氧二萜内酯化合物,与雷公藤碱、雷公藤次碱、雷公藤晋碱、雷公藤春碱、雷公藤增碱和雷公藤明碱等生物碱构成了雷公藤提取物的主要活性成分

这种作用对IDH1突变细胞选择性很高, 施用雷公藤甲素之后,IDH1突变细胞的增殖减少了75%,而正常细胞几乎没有观察到影响

在异种移植的小鼠模型中,雷公藤甲素的施用也大大延长了小鼠的生存期。

这项研究的局限性在于,雷公藤甲素对 Nrf2 的抑制机制尚且没有完全搞清楚,不良反应以及对血脑屏障的渗透能力也需要进一步研究。期待快快搞清楚这些问题,让它走上临床应用吧!

williamhill asia 医学检索资料发现,最近在国际上雷公藤甲素研究非常火热,包括有类风湿,肿瘤,免疫等领域。在肿瘤方面,对于口腔癌,胆管癌等均有一定的研究,同时还发现它能抑制PD-L1的表达。最近一篇发表在Nat Prod Rep上的综述全面介绍了雷公藤甲素在最近20年的主要进展,值得深入阅读[2]。

不过,williamhill asia 医学小编也要提醒,这仅仅是初步研究,与临床仍然有相当的距离。即使雷公藤甲素是IDH1突变的抑制剂,也未必就对所有IDH1突变肿瘤有效,从此前批准的IDH1抑制剂来看,对血液肿瘤效果不错,但是对于其它IDH1突变的实体瘤作用仍然需要研究,另外,能否透过血脑屏障,对中枢肿瘤的作用,也需要考量。总之,一个药物最终上临床可能还需要进行大量的化学改造,剂型改造等。

另外,williamhill asia 医学小编认为雷公藤甲素能否成为靶向药物,目前下结论还为时过早。应该还要通过结构生物学进一步确认,雷公藤甲素是否是直接作用于IDH1突变,与IDH1突变的蛋白存在相互作用,才可能更为可靠。与第一个IDH1抑制剂的化学结构相比,差异还是相当大的。

药监局修订雷公藤中成药说明书
