Blood:短端粒长度可预测MDS患者干细胞移植后的无复发死亡率

异基因造血干细胞移植是治疗骨髓增生异常综合征(MDS)的唯一有潜力的可治愈的治疗方法,但移植相关并发症的风险限制了患者的长期生存预后。由遗传或获得性因素介导的短端粒长度会影响细胞对遗传毒性和复制应激的反应,并可识别移植后有较高毒性风险的患者。

Mikko等人检测了1514位MDS患者移植前的血液样本中的相对端粒长度,并评估了端粒长度与MDS疾病特征以及移植预后的相关性。

端粒长度和年龄、体细胞突变的相关性

经分析发现,较短的端粒长度与年龄大、男性、影响DNA损伤反应的体细胞突变以及移植前更严重的细胞减少症显著相关,但与骨髓母细胞计数、MDS治疗史或既往癌症治疗史无关。

不同端粒长度患者的预后

经显著的临床和异常变异校正后,在1267位≥40岁的患者中,端粒长度最短的四分位数与较差的生存率显著相关(P<0.001),该结果可归因于这类患者的无复发死亡率(NRM)风险较高。较短的端粒对NRM的不利影响与进行移植的患者的合并症无关,偶见于在接受更大强度的预处理的患者中,包括清髓方案和更高剂量的以美法仑为基础的减强度方案。

40岁及以上患者严重急性GVHD预后

在发生严重的急性移植物抗宿主病的患者中,较短的端粒可显著影响NRM,提示较短的端粒长度可能限制了急性损伤后粘膜组织的再生潜力

总之,端粒长度较短的MDS患者,由于毒性大导致存活率较低,可以考虑采取适当措施将移植的毒性作用降至最低,从而改善这类患者的存活预后
