妊娠期体重增加过多会增加妊娠期糖尿病(GDM)的风险,但目前尚不清楚GDM诊断后体重控制是否能改善预后。近日,国际杂志 《Diabetic
Med》上在线发表一项关于“改善妊娠期糖尿病的结果:妊娠期体重增加是否重要?的研究。
研究人员评估了以下问题:(1)怀孕期间的总体重增加(0-36周);(2)早期妊娠期体重增加(0-28周,GDM诊断前);或(3)妊娠晚期体重增加(28-36周,诊断后)是否与母胎结局有关。这项回顾性观察性研究纳入了546名GDM女性,这些女性在一个英国产科中心(2014年10月至2017年3月)分娩了单胎活婴。
研究表明,妊娠期总体重增加与剖腹产 [ n=376;
比值比(OR)1.05; 置信区间(CI)1.02-1.08,P
<0.001],孕龄大(OR 1.08; CI 1.03-1.12,P <0.001)相关。妊娠晚期体重增加较高(28-36周; n = 144)与孕龄较大(OR 1.17; CI 1.01-1.37,P <0.05),器械分娩(OR 1.26; CI 1.03-1.55,P <0.01),每日胰岛素总剂量较高(36周;β系数4.37; CI 1.92-6.82,P <0.001),产后2小时口服葡萄糖耐量试验浓度较高(β系数0.12; CI 0.01-0.22,P<0.05)相关。与GDM诊断后持续体重增加的女性比较,避免显著体重增加的女性在出生后2小时葡萄糖含量低0.7 mmol/ l,且36周时仅需要一半胰岛素/天。早期妊娠期体重增加(0-28周)与妊娠结局之间无显著相关性。
这些研究结果表明,控制妊娠期体重增加应该是GDM诊断后的优先事项,以优化妊娠结局并改善产妇的产后葡萄糖稳态。
