BMJ:慢性阻塞性肺病患者预后预测模型

本研究对COPD患者预后模型进行了详细评估。调查结果表明,大部分在发展过程中存在若干方法上的缺陷,外部验证率较低

近日研究人员就慢性阻塞性肺疾病(COPD)预后模型进行了研究。


本次研究为系统综述及荟萃分析研究,收集截至2018年11月的相关研究。


228个研究纳入本次研究,包含408个预后模型研究,38个外部验证,20个非COPD疾病预后模型验证。408例预后模型建立在3种临床环境下:门诊(239例;59%)、住院(155例;38%)和急诊(14例;3%),其中最常见的终点事件为死亡率(n=209;51%),COPD急性加重风险(n=42;10%)和再入院风险(n=36;9%)。总体上,最常见的影响因素是年龄(n=166;41%)、1秒用力呼气量(n=85;21%)、性别(n=74;18%)、体重指数(n=66;16%)和吸烟(n=65;16%)。408个预后模型中,100个(25%)被内部验证,91个(23%)检查了所开发模型的准确性。对于286(70%)个无已知可用的模型的预测研究中,只有56(14%)模型是通过完整的方程提出的。311(76%)模型采用C统计量进行模型判别。38种模型是外部验证的,但其中只有12种是由完全独立的团队执行的验证。根据Proast的说法,只有七个总体风险偏低的预后模型被确认,包括ADO, B-AE-D, B-AE-D-C,扩展ADO, 升级 ADO, 升级BODE以及Berten模型。对12种预后模型进行了C统计的Meta分析,估计值在0.611-0.769之间。
