Metabolism:非酒精性脂肪性肝病增加了慢性肾脏疾病的发生风险

研究背景:最近的研究调查了非酒精性脂肪性肝病(NAFLD)对慢性肾病(CKD)风险的预后影响。然而,NAFLD对CKD造成事故风险的程度是不确定的。本研究对相关研究进行了Meta分析,以量化NAFLD与CKD疾病发生风险之间的关联程度。
研究方法:从2000年1月1日到2017年8月31日,研究者在PubMed、Scopus和Web上进行了搜索,使用预先定义的关键词来确定大型观测队列研究,随访时间至少为1年,其中NAFLD从生物化学、脂肪肝指数或超音波等方面进行诊断。没有采用肝穿确认为NAFLD的研究不纳入分析(认为是不适合的)。对纳入研究的数据进行提取,运用随机效应模型进行Meta分析。
研究结果:本研究纳入9项观察性分析,共计96595例成年个体(34.1%为NAFLD),主要为亚洲人种。CKD疾病分期至少在3期(即:肾小球滤过率<60 ml/min/1.73 m2,伴随或者不伴随明显的蛋白尿)的4653例患者纳入最后的分析。NAFLD患者发生CKD的风险显著高于非NAFLD患者(随机风险率[HR]为1.37,95%CI 1.20-1.53; I2=33.5%)。NALFD疾病越严重(根据超声和非侵袭性纤维化指标判断)发生CKD的风险越高(n=2项研究; HR:1.50, 95%CI 1.25-1.74; I2=0%)。经超声诊断为NAFLD,且NAFLD纤维化评分越高的患者发生CKD的风险越高(n=1项研究; HR:1.59, 95%CI 1.31-1.93)。敏感性分析并没有改变这些发现。漏斗图和埃格的测试没有显示出明显的发表偏倚。
