研究人员对14671名参与TECOS(评估西格列汀心血管结局的试验)试验的2型糖尿病和CV疾病受试者事后评估了SHEs与复合CV终点(致命/非致命性心肌梗死或中风、住院治疗不稳定型心绞痛、住院治疗心力衰竭[hHF])之间的时间依赖性相关性,并且对这些受试者随访3年。
研究人员发现SHE并不常见,且与西他列汀治疗无关(n=160[2.2%],0.78/100名患者每年,n=143[1.9%],安慰剂为0.70/100名患者每年;风险比[HR]为1.12[95%可信区间为0.89-1.40],P=0.33)。 与没有SHE的患者相比,SHE患者年龄较大,糖尿病病程较长,体重较轻,估计的肾小球滤过率较低,更常见为女性、非白人以及接受胰岛素治疗;并且微量白蛋白尿或大量白蛋白尿更常见。根据临床因素调整的分析显示SHE与主要复合终点事件(1.55 [1.06-2.28],P=0.025)、全因死亡(1.83 [1.22-2.75],P=0.004)、 CV死亡(1.72 [1.02-2.87],P=0.040)风险增加相关。相反,非致命性心肌梗死(3.02 [1.83-4.96],P<0.001)、非致死性卒中(2.77 [1.36-5.63],P=0.005)和hHF(3.68 [2.13-6.36],P<0.001) 与SHE的风险增加相关。完全调整的模型显示SHEs与随后的CV或hHF事件之间没有关联,但CV事件与随后的SHE之间的关联仍然强劲。
