NEJM:低剂量地塞米松对早产风险产妇及新生儿围产期不良结局的影响

在中低收入国家,具有早产风险产妇接受低剂量地塞米松治疗可显著降低新生儿死亡或死产风险,且不增加孕妇细菌感染风险

中低收入国家孕妇及新生儿围产期不良结局风险较高,特别是早产儿,近日研究人员考察了具有早产风险的产妇产前糖皮质激素治疗的安全性和有效性。
 
研究人员进行了一项多国家、随机试验,研究对象是孕26周0天到33周6天之间有早产风险的孕妇。参与者随机接受肌肉注射地塞米松或安慰剂。研究的主要结果为新生儿死亡、死产或孕妇细菌感染
 
研究在孟加拉国、印度、肯尼亚、尼日利亚和巴基斯坦的29家二级和三级医院开展,总计2852名产妇及其3070名胎儿参与。在第二次中期分析后研究停止。地塞米松组1417名新生儿中278名(19.6%)死亡,安慰剂组1406名新生儿中331名(23.5%,相对风险0.84),死产或新生儿死亡分别为393例(25.7%)和444例(29.2%,相对风险:0.88;),产妇细菌感染的发生率分别为4.8%和6.3%(相对风险:0.76)。组间不良事件发生率差异无统计学意义。
 
研究认为,在中低收入国家,具有早产风险产妇接受低剂量地塞米松治疗可显著降低新生儿死亡或死产风险,且不增加孕妇细菌感染风险。
