钙化,在磁共振上到底什么信号?

小编工作之余,翻翻文献,终于找到一些蛛丝马迹,并进行整理,以供各位同行参考学习。有其他不同意见的,欢迎大家留言交流。

一、CT上钙化为什么高?

由于钙化内大多含有钙、磷、硅等成分及钾 、镁 、锌、铁等微量元素, 因此在 CT扫描中显示为高密度。一般CT值在90HU以上。

二、MR上钙化到底可以表现出什么样的信号?

颅内钙化MRIT1 WI和T2WI均呈现出高 、等、低和极低 4 种信号, T1WI 以等信号和低信号较多见 ; T2WI 以低信号和极低信号多见; 综合两种加权图像, 以T1WI 等信号、T2WI 低信号最为常见。

三、什么原因导致了钙化磁共振信号的多样性?

钙化信号的多样性可能来自于钙盐晶体之间的纤维组织或病理组织, 两者的成分 、含量 、分布的不同使钙化病灶表现出信号的多样性 。由于钙盐的 CT 密度极高 ,部分容积效应使其中的少量纤维组织和病理组织不能在 CT 图像上显现出来, MRI 抑制了钙盐的信号, 有助于这些微小病变信号的显现。T1WI 高信号来自于钙化中的某种钙盐化合物, 如磷酸三钙 、氢氧化钙等具有粗糙表面结构及不规则形态的晶体。

四、钙化磁共振信号的差异具体和哪些因素有关?

(1)部位:发生在基底节区的钙化 T1WI 皆呈高信号或等信号,而其他部位病灶则较多地表现出等信号和低信号。

(2)体积:T1WI 信号相似, T2WI 信号与钙化病灶的大小有一定关系 ,小病灶多呈等信号或低信号 ; 而大病灶则显示为低或极低信号——可能与部分容积效应有关。

(3)状态:凝胶状态的 Ca3( PO4) 2 与 Ca( OH) 2 在 MRI T1WI 可以呈现出高信号。

(4)表面形态:T1WI 信号的高低与晶体形态和表面结构有关,粗糙的表面使钙盐呈高信号 , 而较光滑的表面形态使钙盐呈低信号 。不规则的晶体形态使钙盐呈高信号(不规则的晶体形态和粗糙的晶体表面可以结合更多的水分子 , 因而在 T1WI 呈现出高信号) ,而规则的晶体形态使钙盐呈低信号。

(5)浓度:当钙化程度较低时 ,随钙化程度的增高钙化信号逐渐增高 ,钙化浓度达到30%时 ,钙化表现的信号强度最高。随着钙化程度的增加 ,组织内质子密度下降 ,钙化信号又逐渐降低 ,最后可表现为低信号。钙化浓度在30% 左右时 ,钙化可在 T1加权像表现为高信号。

(6)病变性质:肿瘤与非肿瘤性钙化, 两者的MRI信号变化相似。在易发生钙化的病变中 ,如颅咽管瘤 ,少枝胶质细胞瘤等 ,钙化程度迅速增高 ,因而不易检出高信号钙化。对钙化病程较长的病灶 ,如颅内代谢性疾病 ,钙化程度由低到高增加缓慢 ,钙化程度在较长的时间内都比较低 ,就有可能发现高信号钙化。

五、钙化在增强扫描中是否会强化?

钙化本身不会强化,但钙盐晶体之间的病理组织可以强化。
