横断面研究表明阻塞性睡眠呼吸暂停(OSA)与代谢综合征(MetS)之间存在相关性,但是否存在时间关系尚不清楚。近日,呼吸疾病领域权威杂志Eur Respir J上发表了一篇研究文章,研究人员旨在研究OSA对一般人群MetS风险的影响。
这项前瞻性研究结合了两个基于人群的样本:Episono(巴西)和HypnoLaus(瑞士)样本。根据统一标准评估MetS。对Episono样本在基线和随访期间以及HypnoLaus样本基线时进行了多导睡眠图(PSG)检查。OSA根据呼吸暂停-低通气指数定义为轻度(5-15次事件每小时)和中度至重度(≥15次事件每小时)。
研究人员纳入了1853名参与者(年龄平均值±SD为52±13岁,56%为女性),基线时没有MetS。平均±SD为6±1年后,318名(17.2%)参与者发展为MetS。中度至重度OSA与MetS事件独立相关(OR为2.58,95%CI为1.61-4.11),通过调节动脉血氧饱和度<90%的时间百分比,MetS组分从基线到随访的数量增加。Episono样本中的亚组分析证实,基线和随访之间PSG该参数的增加代表了MetS事件的危险因素(每增加10%,OR为1.42,95%CI为1.04-1.95)。
由此可见,在一般人群中通过调节夜间低氧血症,OSA与发生MetS风险增加独立相关。
