《神经病学文献》杂志(Arch Neurol)2012年6月18日在线发表了一项糖尿病(DM)和血糖控制情况与无痴呆老年患者认知功能之间关系的研究结果。这项来自于美国加州大学的前瞻性队列研究发现,DM和血糖控制不佳患者与其认知功能低下有关,且认知功能降低更快,研究提示DM的严重性加速了其认知功能衰退。
研究数据来源于2个社区诊所的健康、老龄化和身体成份研究,共涉及3,069例老年人(平均年龄74.2岁,42%黑人,52%女性),研究对象在基线时接受简易精神状态检查(Modified Mini-Mental State Examination,3MS)及数字符号替换测试(Digit Symbol Substitution Test,DSST),并接受超过10年的选择性随访。收集这些人群基线以及随访时的血糖情况,分别检测1年(基线)、4年、6年和10年空腹全血的糖化血红蛋白A1c(HbA1c)水平。
结果显示,基线时717例(23.4%)患有DM,2352 例(76.6%)未患DM,随访期间159例发生DM。基线时患有DM患者的认知功能低于无DM患者(3MS:88.8 对90.9;DSST:32.5 对36.3;2种测试t = 6.09; P = .001),混合效应模型9年认知功能降低情况表现出类似的情况(3MS:−6.0对−4.5,t = 2.66,P = .008;DSST:−7.9对−5.7,t = 3.69,P = .001)。随访期间发生DM的患者较既往存在DM和随访期间无DM的人群基线和9年认知功能评分呈降低趋势,但与无DM人群之间的差异无统计学意义。经人口统计学数据和临床合并症多因素校正后仍然得到同样的的结果。DM 患者HbA1c水平与认知功能低下相关(3MS: F = 8.2,总体P值 = 0.003;DSST: F = 3.4,总体P值 = 0.04)。
