JAHA:细颗粒物空气污染对青少年心律失常的急性影响

在以人群为基础的青少年样本中,PM2.5暴露与室性心律失常的急性增加有关。PM2.5对室性心律失常影响出现在暴露后2小时内。

心源性猝死(SCD)是一项重大的公共卫生威胁。据估计,在西方国家,SCD占心血管相关死亡的约50%,占所有原因死亡的15%至20%。SCD虽然相对罕见,但在其他健康的儿童和青少年中,包括世界级的运动员,会对他们的家庭造成毁灭性的影响。细颗粒物(空气动力学直径≤2.5μm的细颗粒物[PM2.5])暴露与成年人发生心律失常的风险有关。然而,PM2.5暴露与青少年心律失常之间的关系尚不清楚。

近日,心血管疾病领域权威杂志JAHA上发表了一篇研究文章,为了探究PM2.5暴露与青少年心律失常之间的相关性和时间进程,研究人员分析了322名参加宾夕法尼亚州立大学儿童队列(PSCC)随访的青少年的数据。

研究人员采用浊度计获得了24小时PM2.5浓度。在测量PM2.5浓度的同时,研究人员使用Holter监测仪获得了受试者24小时心电图数据,并从中确定了心律失常情况,包括房性早搏和室性早搏。PM2.5浓度和过早心房收缩/室性早搏(PVC)的数量以30分钟为单位进行总结。研究人员采用负二项模型框架下的多项式分布滞后模型评估PM2.5浓度对房颤和早搏次数的影响。

研究人员发现PM2.5暴露与PVCs数量的急剧增加有关。具体来说,PM2.5浓度每增加10μg/m3与暴露后0.5~1.0、1.0~1.5和1.5~2.0小时PVC数量增加2%(95%CI为0.4%-3.3%)相关。累积起来,PM2.5增加10μg/m3与暴露后2小时内PVC数量增加5%(95%CI为1%-10%)相关。PM2.5浓度与过早心房收缩无关。

由此可见,在以人群为基础的青少年样本中,PM2.5暴露与室性心律失常的急性增加有关。PM2.5对室性心律失常影响出现在暴露后2小时内。
