食管鳞状细胞癌亟待深入研究
食道癌(EC)是全球第七大最常见癌症,也是导致癌症死亡的第六大原因,据2018年全球癌症统计数据,中国每年有477900例食道癌新增病例和375000例食道癌死亡病例。
食管鳞状细胞癌(ESCC)是主要食道癌组织学亚型,特征是其侵袭性的临床过程,并构成了食道癌中预后不良的亚组。
从流行病学的角度来看,食管鳞状细胞癌(ESCC)
的发病率在中国各地区表现出明显的地域差异。与中国其他地区相比,太行山区、新疆以及潮汕地区的食管鳞状细胞癌(ESCC)发病率很高,而中国其他地区的发病率也在迅速增加中。
食管鳞状细胞癌(ESCC)是一种高度异质性疾病,其分子层面分类不清,且临床结果可变,并且尚无可靠的预后生物标志物。食管鳞状细胞癌(ESCC)病因的确切分子机制目前了解的并不多,导致患者的靶向治疗有限、临床管理不足。
尽管已发现了一些潜在的治疗靶点,但ESCC中的突变基因中很少有可在临床上起作用的,目前已批准的治疗晚期ESCC的抑制剂数量非常有限。
因此,非常需要基于全基因组测序(WGS)数据来鉴定新的ESCC癌症驱动基因,并在具有可用临床数据的更大范围的ESCC患者中探索其他预后生物标志物。
研究结论
为了克服这些挑战,研究团队在微针穿刺的福尔马林固定石蜡包埋肿瘤组织中进行了深度全基因组测序,并将508例食管鳞状细胞癌(ESCC)患者的相邻非癌组织与临床随访数据进行了匹配。
该研究发现并鉴定了5个与癌症转移和患者预后相关的5个新的显著突变基因,其中,发现NFE2L2基因可能在食管鳞状细胞癌(ESCC)起到抑癌作用,NFE2L2基因突变可能会损害其抑癌作用,甚至会导致致癌活性,此外,还发现NFE2L2基因突变与食管鳞状细胞癌(ESCC)的不良预后显著相关。
该研究还确定了潜在相关的非编码基因突变,其中,SLC35E2基因启动子区域中的热点突变与患者较差的生存率相关。
总的来说,该研究报告了目前最大的食管鳞状细胞癌(ESCC)基因组图谱数据集,发现了多个与临床相关的编码和非编码基因组突变,可用于开发可用于诊断和治疗的生物标记物。
此外,该研究根据基因组图谱将食管鳞状细胞癌(ESCC)分为三种主要亚型,能够强有力地预测食管鳞状细胞癌(ESCC)患者的预后生存。
