BMJ:意外减重人群的肿瘤风险研究

日常医疗中,体重意外降低人群罹患肿瘤的风险低于2%,但若部分患者出现特定的临床特征,需要警惕肿瘤风险
近日研究人员根据患者的年龄、性别、吸烟状况和临床特征(症状、体征和异常血液测试结果)量化意外的体重降低(WL)对肿瘤诊断的预测价值。
研究人员分析了英国初级保健国家肿瘤登记信息,在2000年1月1日至2012年12月31日期间,总计63973名成年人存在意外的体重降低现象。研究的主要终点为意外的体重降低后6个月内肿瘤诊断。
在63973名意外减重的成年人中,37215(58.2%)为女性,33167(51.8%)为60岁或以上,16793人(26.3%)曾吸烟。908人(1.4%)在6个月内诊断为肿瘤,其中882人(97.1%)年龄在50岁以上。总计分析得到10个与男性意外减重相关的肿瘤风险因素,女性有11个。男性意外减重的肿瘤风险因素中,最低的风险因素为非心脏性胸痛(1.86),最高的风险因素为腹部肿块(6.10),女性意外减重的肿瘤风险因素中,最低的风险因素为腰痛(1.62),最高为黄疸(20.9)。与肿瘤相关的异常血液检测结果包括低白蛋白水平(4.67),血小板异常(4.57),血钙异常(4.28),白细胞总数异常(3.76)和C反应蛋白(3.59)升高。
