肥胖与肿瘤的发生相关,生命早期肥胖与女性癌症风险之间关联的证据是相互矛盾的。人们对这种相关性的确切类型以及这种关联是否独立于成人肥胖仍未明确。
近日,肿瘤学领域权威杂志British Journal of Cancer上发表了一篇研究文章,研究人员通过对前瞻性研究进行了剂量反应荟萃分析,以总结受试者生命早期体重指数(BMI)与乳腺癌、子宫内膜癌和卵巢癌风险之间的关系。
研究人员对Pubmed和Embase数据库进行了全面系统地检索,检索时间截止至2020年6月,以确定并纳入相关的研究。研究人员使用随机效应模型,在年龄≤25岁时,BMI每增加5kg/m2估计总相对风险(RR)和95%置信区间(CI)。此外,研究人员还使用受限三次样条分析进行了非线性剂量反应的荟萃分析。
在初步筛选了33948篇出版文献后,研究人员通过进一步阅读摘要和全文最终确定37项前瞻性研究,并将其纳入该分析。研究人员发现与生命早期BMI每增加5-kg/m2相关的总RRs对于乳腺癌为0.84(95%CI=0.81-0.87),对于子宫内膜癌为1.40(95%CI=1.25-1.57),以及对于卵巢癌为1.15(95%CI=1.07-1.23)。对于乳腺癌,在调整成人BMI后,相关性仍然具有统计学意义(RR=0.80,95%CI=0.73–0.87)。对于绝经前乳腺癌、子宫内膜癌和卵巢癌,剂量-反应曲线表明存在非线性证据。
由此可见,对于生命早期肥胖,该研究的数据支持其与乳腺癌呈负相关,但与卵巢癌和子宫内膜癌风险呈正相关。
