研究者共纳入75例CD患者,进行了一项前瞻性的研究,其中63人完成了12个月的随访。主要的测量IGF-1在基线和10周时的水平,及基线和初始抗TNF-α治疗后12个月的骨骼和肌肉的DXA和胫骨外周定量CT(pQCT)。结果表示为特定于性别的Z分数。
研究结果显示IGF-1 Z评分从-1.0(-1.58,-0.17)的中位数增加至10周的-0.36(-1.04,0.36)(P<0.001)。较小的疾病严重程度和系统性炎症以及更大的雌二醇Z评分(在女孩中)与随时间推移与更大的IGF-1 Z评分显着相关。 DXA全身骨矿物质含量、腿部瘦肉质量和全髋及股骨颈骨矿物质密度(BMD)Z评分在基线时低(p <0.0001vs.参考数据)并在12个月时显着增加(p <0.001)。IGF-1 Z评分超过10周的增加预示着DXA骨骼和肌肉结果以及pQCT小梁BMD和皮质面积的改善。在调整肌肉质量变化后,显着地减弱了IGF-1水平和骨结果之间的关联。
