IBD: 生命早期抗生素暴露和克罗恩病发病风险的相关性分析

研究人员通过国家数据库确定了 1995 年至 2009 年在丹麦出生的所有儿童,并跟踪他们从出生到死亡或诊断为CD。使用 Cox 回归,研究人员评估了生命第一年抗生素暴露与随后的 CD 风险,以及通过分析性别、城市化程度、出生顺序、出生年份、分娩途径、胎龄、怀孕期间吸烟、出生第一年非甾体抗炎药的摄入量进行进一步比较。
在中位 9.5 年的随访时间中,979,039 名儿童中有 208 人被诊断出 CD。在生命的第一年使用抗生素与更高的 CD 风险相关(调整后的风险比,1.4;95% 置信区间 [CI],1.1-1.8),≥6 个疗程的抗生素风险最高(调整后的风险比, 4.1; 95% CI, 2.0-8.5)。CD 家族史并未改变这些风险关联。出生后第一年接触抗生素的儿童在 11 岁时患 CD 的累积风险为 0.16‰(95% CI,0.11‰–0.22‰),而出生后第一年未接触过抗生素的儿童在 11 岁时患 CD 的累积风险为仅为 0.11‰(95% CI,0.08‰–0.15‰)。
