CGH: 急性胰腺炎后他汀类药物使用与糖尿病发展的关系

胰腺炎可能会导致糖尿病,急性胰腺炎疾病发病的时候,很多患者都会出现血糖的升高,主要是应激导致血糖水平的升高,当患者胰腺炎如果治疗的比较及时,痊愈后,血糖水平也会逐渐降到正常。
胰腺炎后糖尿病(PPDM)是一种糖尿病 (DM) 的亚型,主要是由由急性胰腺炎(AP)或慢性胰腺炎引起。一项对1102名首次出现AP患者的系统评价发现,高达15%患者在AP发作后的12个月内患糖尿病。在台湾进行的两项研究表明,患有AP的患者患DM的风险高出 2 倍,有些研究表明,不是胰岛细胞的破坏而是其他机制是造成 PPDM,潜在机制包括慢性低度炎症、脂肪细胞的脂肪分解、冠状动脉的病变等。
此外,他汀类药物已被证明具有多效性,虽然他汀类药物相关的药物性胰腺炎也不罕见,但是最近的流行病学研究表明表明他汀类药物可以降低AP的风险。虽然他汀类药物已被证明会导致易感人群新发 DM 的风险略有增加,但是没有研究其预防 PPDM的作用。
研究人员通过收集了118479名患者数据用于评估他汀类药物对首次出现AP时既往没有 DM 的患者的影响。定期使用他汀类药物被定义为在AP前一年至少80%的时间使用他汀类药物处方药。主要观察结果定义为PPDM的出现。研究人员同时构建了一个倾向评分并应用治疗加权的逆概率来平衡组间的基线特征。使用Cox比例风险回归模型评估了PPDM 的危险因素。
本项研究的中位随访时间为3.5年,常规他汀类药物使用者 PPDM 的5年累积发病率为 7.5%(95%[CI],6.9%-8.0%),而不使用他汀类药物的患者的 PPDM 的5年累积发病率12.7%(95%CI,12.4 %-12.9%)。与不使用他汀类药物的人相比,经常使用他汀类药物的人发生 PPDM 的风险降低 42%(OR,0.58;95% CI,0.52-0.65; P< .001)。不定期使用他汀类药物的PPDM风险降低15%(OR,0.85;95% CI,0.81-0.89;P< .001)。低、中和高剂量的他汀类药物也有类似的益处。
本项研究证实他汀类药物的使用降低了急性胰腺炎后发展为 DM 的风险。但是需要进一步的长期随访前瞻性研究来研究他汀类药物对急性胰腺炎和预防 PPDM 的影响。
