Neurology:脑出血复发影响因素研究

研究认为磁共振确证的脑微出血情况对脑出血幸存者再出血具风险有显著影响,有望作为脑出血幸存者预后的预测指标

近日研究人员对脑出血(ICH)幸存患者的脑出血复发风险进行了评估。

本次研究对前瞻性队列性脑出血临床研究结果进行荟萃分析,在脑出血后进行血敏感性脑磁共振的患者参与本次研究。研究依据患者MRI检测的脑微出血(CMBs)的存在、分布以及数量进行分层。研究人员对每个临床试验患者的脑出血复发率进行统计并比较CMB存在与否、CMB分布(大脑淀粉样血管病【CAA】)以及严重情况(出血数量:1,2-4,5-10以及>10个出血位点)对脑出血复发的影响。

研究对10个临床试验进行了荟萃分析,总计1306名患者,其中325人为大脑淀粉样血管病相关脑出血患者,981人为非大脑淀粉样血管病相关脑出血患者。统计发现,CAA相关ICH患者与非CAA相关ICH患者相比,年脑出血复发风险提高(7.4%, 95% CI 3.2–12.6 vs 1.1%, 95% CI 0.5–1.7; p = 0.01)。对于CAA相关脑出血患者,存在多个CMB位点相比于无出血点患者,1-3年的脑出血复发风险如下:2-4个出血点OR,3.1 (95% CI 1.4–6.8; p = 0.006), 5-10个出血点OR,4.3 (95% CI 1.8–10.3; p = 0.001), 超过10出血点OR,3.4 (95% CI 1.4–8.3; p = 0.007)。对于非CAA相关脑出血患者,仅超过10个以上的出血点会增加脑出血复发风险(OR 5.6, 95% CI 2.1–15; p = 0.001)。仅1个CMB出血位点不会导致脑出血复发,无论CAA相关或不相关脑出血患者。
