Eur Heart J:恩格列净和尿酸对射血分数降低心力衰竭患者的作用

高尿酸血症在心力衰竭中很常见,是疾病晚期严重程度和死亡率增加的独立预测因子。恩格列净引起快速和持续的SUA水平下降和与高尿酸血症相关的临床事件。恩格列净对主要结局的益处独立于SUA。

钠-葡萄糖共转运体-2抑制剂恩格列净可降低射血分数降低的心衰患者心血管死亡或因心衰住院的风险。恩格列净可降低血清尿酸(SUA)水平,但这种作用在心衰患者中的相关性尚未明确。

近日,心血管领域权威杂志Eur Heart J上发表了一篇研究文章,该研究旨在探讨恩格列净对SUA水平的影响以及与SUA相关的治疗效果。

研究人员对3676例EMPEROR-Reduced试验患者(占该研究队列的98.6%)进行了SUA与心血管死亡或因心衰恶化住院治疗的复合主要结局、其成分和全因死亡率之间的相关性研究。研究人员并将SUA作为连续变量,对高尿酸血症(女性SUA>5.7mg/dL,男性SUA>7.0mg/dL)和SUA三分位数患者亚组恩格列净的治疗效果进行了关联分析。

高尿酸血症患者占53%,且无性别差异。SUA升高(最高三分位数,平均SUA为9.38±1.49mg/dL)与心力衰竭的严重程度和最差结局相关[综合结局风险比(HR)为1.64(95%可信区间(CI)为1.28-2.10);心血管死亡率风险比为1.98(95%CI为1.35-2.91);多因素校正分析显示,与最低三分位数组相比,全因死亡率风险比为1.8(95%CI为1.29-2.49),均P<0.001)。使用恩格列净治疗4周后,SUA降低(与安慰剂相比:−1.12±0.04mg/dL,P<0.0001),并在整个随访期间保持较低水平,所有预先指定的亚组也有类似的降低。恩格列净降低临床相关高尿酸血症(急性痛风、痛风性关节炎或开始抗痛风治疗)事件32%[HR为0.68(95%CI为0.52-0.89),P=0.004]。恩格列净对主要终点的有益作用独立于基线SUA[HR为0.76(95%CI为0.65-0.88),P<0.001]和4周时SUA的变化[HR为0.81(95%CI为0.69-0.95),P=0.012]。SUA和治疗效果之间的相互作用表明,恩格列净对SUA升高患者的死亡率(心血管和全因死亡率)存在好处(相互作用的P分别为0.005和0.011)。
