长期熬夜会猝死?心脏昼夜节律需关注

随着人们对生物钟基因的认识和研究的不断加深,生物节律在血管病理生理过程中发挥的作用将逐渐被解密,有理由相信,干预生物节律将为预防和治疗心血管疾病提供重要的理论依据。

无论是生活所迫夜班不断的打工人,还是辛苦一天放松熬夜的修仙派,长时间的昼夜节律紊乱,往往会给身体带来一系列伤害,心源性猝死是其中最为严重的后果,其中重要的原因在于打破了人体生物钟的平衡。地球每24小时自转一次,地球上几乎所有的生命都有昼夜节律,来适应地球自转引起的变化。2017Jeffrey C. Hall Michael RosbashMichael W. Young因发现昼夜节律在生理和疾病中的重要作用而获得诺贝尔生理学、医学奖。昼夜节律的混乱会提高多种心血管疾病的发病率,诸如心肌梗塞、心源性猝死和室性心律失常等,心脏也有其独特的昼夜节律,而这种节律是由分子生物钟机制控制的,该机制发生障碍时,将会对心脏产生影响。

针对于心脏昼夜节律方面早有研究,生物钟的周期性振荡是通过调控几种转录/翻译反馈回路实现的(图1)。在该反馈回路中,ROREREV-erbAαE-box的核心功能最为关键,他们通过调控几种生物钟基因的转录,包括BMAL1CLOCK等,实现对下游基因PERCRY等依赖BMAL1CLOCK转录的基因的调控作用。而PER/CRY异源二聚体浓度一旦达到临界水平,PER/CRY异源二聚体又可进入细胞核,转而抑制BMAL1/CLOCK的活化和自身转录,并开始一个新的调节周期。这个反馈循环致使BMAL1/CLOCKPER/CRY的表达振荡遵循昼夜节律模式。在小鼠身上的研究总结如表1所示。

1. Clock机制紊乱:来自动物研究的结果

2022117日,德克萨斯州休斯顿贝勒医学院、温州医科大学育英儿童医院、中国医学科学院和北京协和医学院等机构在美国心脏协会期刊Circulation预印本平台联合发表了一篇名为“Myocardial Rev-erb-Mediated Diurnal Metabolic Rhythm and Obesity Paradox”的文章(图2),该文章通过基因处理和饮食改善,研究生物钟关键控制组分REV-erbAα/β在动物和人类心脏代谢节律方面的功能,以期探索扰乱昼夜节律对心脏疾病的影响。

在该项研究中,以小鼠和人类心脏为实验对象,分析了在不同处理条件下的小鼠正常昼夜节律时期内,心肌细胞中的REV-erbAα/β的表达以及代谢产物的变化。

结果如下:

1Rev-erbα/β基因仅在睡眠时间高表达,其活性与脂肪和糖代谢有关。

2、心肌细胞中的REV-erbAα/β调控正常代谢节律,从而使心肌细胞在动物休息阶段倾向于以脂质作为能量来源。敲除REV-erbAα/β后,小鼠心肌细胞丧失正常代谢节律,在小鼠休息阶段降低了使用脂质的能力,并导致进行性扩张型心肌病和致命的心力衰竭。

3、人类心脏昼夜节律紊乱可能在扩张型心肌病和心力衰竭中起因果作用。

4、通过补充游离脂肪酸可对睡眠周期中的心脏起保护作用

5、在昼周期抑制脂肪酸氧化,在夜周期抑制糖酵解的计时疗法策略可能对因昼夜节律紊乱导致的心力衰竭患者有益。

此项研究的负责人表示:该项研究探索了通过药理学调控脂肪酸和糖代谢以改善心脏疾病的可能性,虽然药物可以帮助恢复已改变的代谢途径,但更为重要的是让药物与相应代谢途径的内部昼夜节律保持一致。如果这些药物的给药方式与昼夜节律不同步,那么治疗并不能改善心脏状况。

随着人们对生物钟基因的认识和研究的不断加深,生物节律在血管病理生理过程中发挥的作用将逐渐被解密,有理由相信,干预生物节律将为预防和治疗心血管疾病提供重要的理论依据。
