Hypertension:血压水平与痴呆症风险

较低的血压个体中可能的阿尔茨海默病和可能的VaD风险不同。尽管接受抗高血压药物治疗的血压较低的个体患痴呆症的风险似乎更高,但这一发现可能会受到合并症的影响。

近日,心血管权威杂志Hypertension上发表了一篇研究文章,研究人员通过抗高血压治疗和合并症调查了血压与痴呆亚型风险之间的关联。
研究人员使用了2009年至2012年的韩国国民健康保险服务-健康筛查的数据库,对4522447名60岁以上无痴呆病史的成年人进行了分析,并进行了平均5.4年的随访。个人根据他们的基线收缩压(SBP)和舒张压进行分类;以SBP为130-140mmHg和舒张压80-90mmHg作为参考组。
在SBP≥160mmHg和较低SBP的组别中,整体痴呆和可能的阿尔茨海默病的风险显著更高。无论是否使用抗高血压药物或合并症,这些U形关联都是一致的。在较低SBP组别中,可能发生血管性痴呆(VaD)的风险并未升高,并且随着SBP的增加而逐渐增加。尽管在未服用抗高血压药物或没有合并症的个体中SBP与可能的VaD风险之间存在线性关联,但在服用抗高血压药或合并症的个体中存在U形关联。舒张压与可能的阿尔茨海默病或可能的VaD风险之间的关联模式与SBP相似,除了服用抗高血压药物的个体可能发生VaD的风险。
总之,较低的血压个体中可能的阿尔茨海默病和可能的VaD风险不同。尽管接受抗高血压药物治疗的血压较低的个体患痴呆症的风险似乎更高,但这一发现可能会受到合并症的影响。
