近日,神经病学领域权威取杂志Neurology上发表了一篇研究文章,研究人员旨在评估多发性硬化(MS)患者身体合并症与残疾进展之间的关系。
研究人员在加拿大的两个省份通过卫生行政和临床数据库进行了一项回顾性队列研究。参与者包括1990年至2010年期间发病的MS患者,这些患者在MS发病日进入队列。合并症状态是通过卫生行政数据的验证算法来确定,并在研究进入前1年和整个研究期间进行测量。结局为每次诊所就诊时记录的扩展的残疾状态量表(EDSS)得分。在调整了性别、年龄、进入队列年份、使用疾病缓解药物、病程和社会经济状况后,研究人员使用广义估计方程来评估随着时间的推移身体合并症与EDSS评分之间的关系。荟萃分析被用来估计2省的整体效应。
研究人员确定了3166例MS患者。身体合并症与残疾有关;每增加一项合并症,EDSS评分平均增加0.18(95%可信区间[CI]为0.09-0.28)。在特定的合并症中,缺血性心脏病(IHD)或癫痫与较高的EDSS评分相关(IHD为0.31,95%CI为0.01-0.61;癫痫为0.68,95%CI为0.11-1.26)。
由此可见,身体合并症与MS残疾进展明显加快有关。需要适当的合并症治疗以优化患者结局。
