腰背痛:尝试瑜伽吧!疗效等同理疗

以前的研究表明,瑜伽可以改善疼痛和机体功能,减少药物的使用。例如,一个2013的荟萃分析显示,短时间内小到中等程度的瑜伽就可改善长期背部疼痛相关的无力。研究还表明,PT可以有效治疗患者的腰背痛。

PT是一种常规治疗,是慢性腰背痛患者最常见的非药物治疗方法,Saper医生说。初级保健医疗中大约有22%的患者是低程度的腰痛患者,并且通过PT治疗。

在这项新的研究中,研究人员招募了320名来自波士顿地区的社区卫生中心的成年患者,这些患者有慢性背部疼痛,没有明显的解剖原因,如椎管狭窄。患者主要是有较低的教育水平的白人和低收入者。

Saper医生介绍说,这些患者有“相当高”的疼痛评分(平均7-10分,根据疼痛量表)和就他们的疼痛情况而言是“轻微影响功能“的。近三个季度有使用疼痛药物,约20%的曾服用阿片类药物。

患者被随机分配成三组:瑜伽组,PT组,或教育组。
瑜伽组的患者每周接受一次维持75分钟的瑜伽课,并给他们一张DVD要求他们在家里练习。PT组有15次一对一且每次维持60分钟的训练,其中包括有氧运动。教育组收到一本关于背部疼痛的综合性的书。

瑜伽和PT组课程持续了12周,之后随访52周。

研究的主要问题是,12周的治疗,瑜伽是否不劣于PT。

Saper医生介绍说,“研究表明,机体功能(Roland Morris残疾问卷评分)、瑜伽和PT是完全相同,即非劣性的,但是,他们与12周的教育组间的差异不是很大。”

参与者中至少有30%的比例疼痛减轻了,“根据Roland[Morris残疾问卷评分],瑜伽组减轻的有48%,PT组减轻的有37%,教育组减轻的有23%,”Saper医生说。

在基线时,约有70%的参与者使用药物治疗。在12周,这样的使用在瑜伽和PT组下降了约20%,而在教育组并没有改变。

对于轻微的、短暂的腰背部疼痛加剧,瑜伽被证明是安全的。

除了低粘附率,这项研究的另一个限制可能是,其结果可能无法推广。”这是一个非常结构化、标准化的瑜伽课程,”Saper医生说。”如果病人们去街上的瑜伽工作室,医生们不知道病人将会怎么做。”需要更大的研究,制定更好的策略,以提高依从性。
