目的:目前的研究在新南威尔士州招募的267,153名男性和女性参与者中前瞻性地检查了45和UP研究中的潜在风险因素,这是一项基于澳大利亚人群的大型研究。这项研究的目的是评估选定的社会人口、行为和健康相关因素是否与PC诊断的风险更高或更低相关,同时解释PSA测试可能造成的混淆。
方法:男性参与者来自Sax Institute的45岁及以上研究(澳大利亚),从2006至2009年间招募。调查问卷和来自澳大利亚健康记录联系和服务中心的相关管理健康数据被用来确定事件PC、医疗保健使用、前列腺特异性抗原(PSA)检测报销和二甲双胍和良性前列腺增生症(BPH)处方的分配。多变量COX和联合COX回归分析被用来检验癌症扩散的相关性,并对各种混杂因素进行了调整。
结果:截至2013年底,在107,706名符合条件的男性中,有4257人患上了偶发PC。PC诊断的风险随以下因素增加:PC家族史(与无癌症家族史比较;调整后的HR=1.36;95%CI:1.21-1.52);父亲和兄弟诊断为PC(与非癌症家族史比较;调整后的HR=2.20;95%CI:1.61-2.99);严重的下尿路症状(与轻度比较;调整后的HR=1.77;95%CI:1.53-2.04)和输精管结扎术(与无比较;调整后的HR=1.08;95%CI:1.00-1.16)。对于BPH(HR调整后为0.76;95%CI:0.69-0.85)和二甲双胍(HR调整后为0.57;95%CI:0.48-0.68),开处方(与不开处方相比)可降低PC风险。输精管结扎术(调整后的HR=1.28;95%CI:1.06-1.55)和肥胖(与正常体重相比;调整后的HR=1.31;调整后的95%CI:1.01-1.69)增加了晚期PC的风险。

结论:输精管结扎术和肥胖与晚期PC的风险增加有关。与BPH和糖尿病相关的局部性和进展性PC的风险降低值得研究。
