JAHA:老年人降压药物依从性和心血管疾病的风险有何关系?

降压药依从性能降低心血管疾病(CVD)的风险;很少有研究探讨老年人中是否存在这种相关性。
近日,心血管疾病领域权威杂志JAHA上发表了一篇研究文章,研究人员在医疗费服务受益者中在2008年至2009年期间新诊断为高血压并开始抗高血压治疗的66岁至79岁的患者中评估了这种相关性(n=155597)。研究人员使用的比例亚分布风险模型计算了随访期间药物覆盖天数的比例(PDC)来评估降压药依从性与复合CVD结局之间的相关性,复合CVD结局包括首次发生的致死/非致死性急性心肌梗死、缺血性心脏病、脑卒中/短暂性脑缺血发作和心力衰竭。
在随访期间(中位数为5.8 年,798621人次每年),研究人员记录了47198例心血管事件。在受益者中,分别有60.8%、30.3%和8.9%的患者PDC≥80%、40%-79%和<40%。对于PDC≥80%、40%-79%和<40%的患者CVD事件的粗略发生率分别为40.1(95%可信区间2为40-40.1),93.9(93.8-93.9)和98.1(98.1-98.2)每1000人每年。调整后的心血管事件的风险比为1(以<40%为参考组),对于PDC在40%-79%的患者为1(0.97-1.03),对于PDC≥80%为0.44(0.42-0.45)(P<0.001)。剂量反应分析表明PDC与CVD事件风险之间存在非线性关系,保护作用≥80%。PDC与缺血性心脏病、脑卒中/短暂性脑缺血发作和心力衰竭之间的相关性在很大程度上与CVD事件和不同人群相一致。
